Below is important information about your appointment so please read carefully.

FOR PATIENTS WHO HAVE APPOINTMENTS SCHEDULED FROM JUNE 5TH THROUGH JUNE 21ST

Our office will contact you a few days prior to your originally scheduled appointment to reschedule for a date after June 22nd. We know your time is valuable, and we will do everything to get you and or your family members rescheduled.

FOR PATIENTS WHO HAVE APPOINTMENTS SCHEDULED FROM JUNE 22ND AND LATER

Our office will still reach out to confirm your upcoming appointment via telephone call or text a few days before your appointment.

NEW PROCEDURES FOR ALL PATIENTS

You will see some important changes to our office and the way we see patients. These measures have been enacted to help protect the health of everyone. Please note the following instructions for your upcoming appointment at Dr. Jonathan Stone’s Comprehensive Family Dentistry office:

1. All patients will complete The American Dental Association (ADA) patient screening questionnaire before you can be treated. You can complete the form prior to your arrival by following this link – Wellness Screening

2. All patients will be required to wear a mask prior to entry until they are seated for exam, no exceptions.

3. Our waiting room will not be available. We have setup a text messaging system that will alert you when to come upstairs for your appointment. We request you wait outside of the building until you receive the message to enter. This procedure will be detailed during your appointment confirmation phone call mentioned above. If you do not have a cell phone, we will make alternative arrangements.

4. All family members not receiving treatment are requested to wait outside of the building unless other arrangements have been made prior.

5. External temperatures will be taken upon arrival.

6. Hand sanitizers will be available for use upon entry and departure.

As you know, my staff and I greatly value your time. We request you make EXTRA efforts to arrive on time, and we will try very hard to see you on time!

Since we have to reduce the number of patients seen in a day, we urge you to provide us 24 hours notice if you need to cancel an appointment so we can provide that time to another patient.
